  EHRS Provider Certification

diagonal image
The Community Care Program, administered by the Illinois Department on Aging, offers a statewide network of services to persons age 60 and over. These services offer eligible older adults of Illinois the opportunity to remain in their own homes as an alternative to inappropriate or premature institutionalization in a nursing facility.

Emergency Home Response Service Certification
The Illinois Department on Aging is soliciting applications for certification in the provision of Emergency Home Response Service (EHRS) under a statewide demonstration project.

Emergency home response service (EHRS) is defined as a 24-hour emergency communication link to assistance outside the client’s home for clients based on health and safety needs and mobility limitations. This service is provided by a two-way voice communication system consisting of a base unit and an activation device worn by the client that will automatically link the client to a professionally staffed support center. The support center assesses the situation and directs an appropriate response whenever this system is engaged by a client. The purpose of providing EHRS is to improve the independence and safety of clients in their own homes in accordance with the authorized plan of care, and thereby help reduce the need for nursing home care.

Download the Frequently Asked Questions (PDF) regarding Emergency Home Response Service Certification.

Because applications are being solicited under an all willing and qualified process, there is no deadline for submission of applications or limit to the number of contracts which could be awarded, based upon a successful response to the service standards for the EHRS program. Applications will be evaluated as they are received and applicants will be notified, in writing, of the results of their certification application for emergency home response service. Faxed transmission of applications will NOT be accepted by the Department.

To apply, download both the Standards (PDF) and the Application for Certification for Providers of Emergency Home Response Service (PDF) then follow the instructions.
