Case Manager Recertification
Illinois Department on Aging (IDoA) Certified Case Managers whose recertification is due before July 1, 2008, may attend the 2008 Mental Health and Aging Conference at no cost.
IDoA Certified Case Managers attending the conference to obtain recertification must mark "Recertification" on the registration sheet as well as their eight digit certification number to attend the conference at no cost.
An IDoA Case Manager Recertification form will be available at the Conference Registration Desk. Four sessions must be attended and verified on the recertification form by session speakers or hosts for the case manager to receive certification.
Completed Recertification forms must be returned to the registration desk by the end of the conference to be valid. The resulting certification cards will be mailed to the attendee’s agency following the conference.
Anyone registered for recertification who fails to return a valid Recertification form, will be charged full conference price.
